DO NOT SUBMIT this form if paying by check—registration will not be recorded. The deadline for Pre-registration is November 1, 2006. On-site registration is required after that date. Please note MESA’s refund policy listed below.

8. Refund Policy
For non-participants, MESA will give a full refund of the pre-registration amount provided that the request for refund is made by no later than the pre-registration cut-off date of November 1. After that date no refunds will be issued. If you are uncertain whether or not you will be able to attend the meeting, please plan to register on site. Meeting participants should be pre-registered for the meeting; refunds for participants will be given only to those who request one by August 15, 2006. 

In order to process your registration, you must acknowledge that you have read and accept MESA's refund policy. If you do not wish to abide by MESA’s refund policy, we ask that you register at the meeting.
